Please fill out all fields above and carefully ready all the conditions below. Once you have ticked the agree to the conditions please submit this form. Once the form is received one of our service team will email you back with a job RMA number. Please print out a copy of the emailed order request form with the RMA number, sign and include when shipping the unit to Si Clean Energy for repairs. Please Note if only dowloading the form: No repairs will commence unless the form has been filled in, signed and returned with the unit TERMS AND CONDITIONS The fee for Si Clean Energy to assess your item is $150 +Gst Equipment that is deemed to be a (no fault found or a non Si Clean Energy warranty fault will also incur this standard assessment fee (stated above). All repair charges will be discussed with you and quoted before commencement of the repair. FREIGHT NOTES Freight to Si Clean Energy is the responsibility of the client unless the frieht box above is filled in / return freight will be quote and arrange by us & invoiced to you, unless notified that prior arrangements have been made. If you are having difficulties finding a completive quote for the freight and wish to use our freight account to reduce cost sending the unit we can arrange a quote using our freight accout (see above). If you select this option we request that the service fee $150.00 + GST and the quoted fright charges be paid upfront so as to reduce admin time. Once paid we will organsie for the collection of the parcel from your address. When requesting this option please also include the packed weight and LxWxH in mm for the packed item to be sent in the feild above. Please note that insurance for the item is not included in this cost. If you request insurance for the freight extra cost you will need to specifie and it will be include in the quote. Please be advised that final payment and collection of the the unit once repaired or assessed must be completed within 14 day's. If no response is received within 60 days from the date of arrival at Si Clean Energy's service department the repair request agreement will be considered as expired and Si Clean Energy retains the rights of disposal of the unit in an environmentally friendly way.

Si Clean Energy will not be liable for any damage to item's during freight when using our account and we suggest that you make sure the unit it is packed with care and in a box that is suitable for being transported with a freight company.
